Occupational Therapist – Community Reablement

Community OT Team – Pathway 1 Reablement (Wolverhampton)
A service in Wolverhampton is seeking an experienced Senior Occupational Therapist to join their Community OT Team supporting Pathway 1 Reablement. This is a 12-week role working 3 days per week, focused on supporting individuals in the community to regain independence following hospital discharge.

Main Duties:

Provide occupational therapy assessments within the Pathway 1 Reablement service
Support individuals to regain independence at home following hospital discharge
Carry out functional assessments and develop goal-focused reablement plans
Recommend appropriate equipment, adaptations, and interventions to promote independent living
Work collaboratively with multidisciplinary teams including health and social care professionals
Manage and prioritise a community-based caseload
Support discharge pathways and help prevent hospital readmissions
Provide professional advice and guidance to colleagues within the service
Requirements:

Diploma/Degree in Occupational Therapy
HCPC Registration
Previous experience working in community or reablement services
Experience completing functional assessments and supporting independence at home
Ability to work independently while contributing to a multidisciplinary team
Strong communication, organisational, and clinical decision-making skills
Ability to travel within the community as required
