C4 Care are looking for an experienced and organised Care Coordinator to join their growing healthcare team. This is an exciting opportunity for a dedicated Care Coordinator who is passionate about delivering high-quality, person-centred care and supporting the smooth day-to-day coordination of care services.

As a Care Coordinator, you will play a key role in ensuring care packages are staffed effectively, rotas are managed efficiently, and both clients and care staff receive excellent support. The successful Care Coordinator will work closely with management, carers, service users, and external professionals to maintain high standards of care delivery.

Key Responsibilities of the Care Coordinator:

* Coordinating care staff rotas and shift allocations

* Ensuring continuity and quality of care for service users

* Supporting recruitment, onboarding, and compliance of care staff

* Liaising with clients, families, healthcare professionals, and care workers

* Managing last-minute staffing changes and emergency cover

* Monitoring staff availability, attendance, and performance

* Maintaining accurate care records and compliance documentation

* Supporting with care planning and person-centred service delivery

* Ensuring all care services operate in line with CQC standards and company policies

Requirements for the Care Coordinator Role:

* Full UK Driving Licence

* Previous experience as a Care Coordinator, Care Scheduler, or similar healthcare coordination role is essential. Unfortunately, senior care experience alone will not be suitable on this occasion, as this is a fast-moving new contract with limited time available to train candidates without existing coordination experience.

* Strong understanding of domiciliary care or supported living services

* Excellent communication and organisational skills

* Ability to work in a fast-paced care environment

* Good IT skills and confidence using rota and care management systems

* Knowledge of CQC compliance and care standards

* Ability to build positive relationships with care staff and clients
